阴森恐怖的森田医院，猪倌院长森田健三（菅田俊 饰）下落不明，猪倌4名护士先后辞职。年轻人佐藤文仁（窪塚洋介 饰）因病住进了这个阴冷所在。某晚，文仁的朋友武史（松尾政寿 饰）前来探病，却意外遭遇一个全身赤裸的绝世美女。武史将这个自称富江（宝生舞 饰）的无助女子带回自己的住所，他深深迷恋拥有神秘气质的富江，在此过程中性格也发生巨大的变化。与此同时，森田院长的女儿由美（山口紗弥加 饰）试图调查父亲的行踪，结果发现医院内的医护人员共同坚守着一个不可告人的秘密。这一切都与此前的一起手术以及神秘的富江有关……本片根据日本著名恐怖漫画家伊藤润二的同名系列漫画改编。
